The last time I connected in SFO (SAN-SFO-PHL) and made a change from UA to US metal, there was a UA rep there to take me out a back door to a bus which brought me over to the other terminal.

FWIW, when I was flying all UA JFK-SFO-NRT, I had the same treatment with the bus, taking me from the domestic to the International terminal.
