I ran into the same issues and sent the form. After no action for one month, I called the Altitude line. The agent walked through each flight and double checked with their supervisor. The credits were then added back into my account. I think they're issues post IT Amadeus. In the past, they were added much more quickly.
I was on the Altitude line and the agent went and talked to her "Help Desk." No dice. She just sent an escalation.
I guess I will call back and tell the agent to speak with supervisor.
