All the gates have the readers now (didn't at the beginning), sometimes they are tempermental and have problems reading the bar code, but not that often so I won't say its a one-time thing but it shouldn't occur more than a few time here and there. If I talk to the CSRs in the RCC or the GA, they'll often give me a new pass without asking once they notice the paper ones.
I definitely prefer to go the airport with pass in hand these days.
