Could someone please PM me or post a copy of the back of a current pre inspection damage form?

The back lists predetermined prices for damage for
stone chip
windshield
wheel
tire price 14in 15in 16in etc

I usually carry one, and I know the 'report and pay before contract is closed' price is MUCH cheaper and faster than a traditional damage report and claim. If I recall it was very cheap like $75 for tires or $150 for windshield.
