I have changed under a blanket not just on QR but also on EY (window seat, no door but somewhat recessed). It's not a big deal if you do it when the cabin lights are dimmed.
If you need more privacy, select the lavatory with a baby changing table. It is usually clean, especially compared to the floor. You can sit on it and lift your feet way up.
