I find the topic of fees to upsell/upfare a ticket to be confusing for many including some AC agents. I don't always get the same answers, but sometimes, agents do go in search of help or clarification.
I spoke to an agent recently. She seemed well informed so I asked a few questions.
Upsell/Upfare to a Higher Cabin
If you bought a Y or PE ticket, for example, and want to move up to J, the change fees are waived. You just pay the fare difference. You can change date of travel and destination. As always, the most restrictive fare rules apply.
The only issue that may come up is how different the destination is that you are changing to. For eg. I've done these with tickets to one destination in Asia and switched to another one in Asia, but this agent seemed to think it would be challenging or not possible if one were to change NRT to LHR. If someone here has made a change similar to this, perhaps they can post.
Upsell/Upfare within the Same Cabin, Within the Same Fare Family
Here is where it can be confusing.
If you have a Standard Y and want to move to Comfort Y,
you do have to pay a change fee as well as the difference in fares. You cannot change dates or destination. You also have to pay the change fee and fare difference if you move from PE lowest(N, E) to PE Flexible (O).
An example would be if someone has a Standard Y ticket (perhaps paid for by an employer) and wants to pay to move to a Y fare where less credits are needed to upgrade. I also know this will be complicated by the alphabet soup of branded fares. (I had a Comfort K fare that needed 8 credit to upgrade, but the higher priced Comfort Fares needed less.)

As I've mentioned in previous posts, I've only done upsells from PE to J and they were easy and I understood the rules. I think the challenge here is more for those who want to stay in the same cabin or same fare family. The agent I spoke with said ACpedia numbers and info change all the time so if one wants to upsell/upfare, be clear with the agent exactly what you'd like to do. I would hope they would all make the effort to provide the right info but as always, YMMV.
