I don't know the number of overall late check out refusals, but I've been denied enough times that after the first refusal I just ask for the hotel GM who usually mumbles something about the "new" FD person not knowing. There is no monetary or point payout to the guest if the late checkout guarantee is refused. The joke is on us.
