BAL was Friendship International Airport and was renamed to BWI in 1973. It took over from Baltimore Municipal Airport, which closed in 1960.

Including name changes (as opposed to code changes)? - I flew from the old Greater Pittsburgh International Airport, now Pittsburgh International Airport, to a few airports that no longer exist/no longer serve commercial flights. Stapleton and Groton come to mind. I'm sure there are several others that have just changed names.
