Lufthansa knows.
Lufthansa has a program where doctors can enroll. The doctor has to provide documentation of a current medical license and Miles and More member number. In turn, the doctor gets a book about airline medical emergencies and a few points as a one time enrollment bonus. The airline then knows when there is an enrolled doctor on their flights.
All this info is second hand from a LH passenger/doctor and from around 2015.
