Regarding (1): The original Economy ticket was a non-refundable fare. So, in case of no upgrade, there will be a change fee associated with it. But a new ticket has been issued after the mileage upgrade. So I am not sure what happens in this case.

According to the following AA site, it seems the copay is not refundable. But I am not sure if I understood it correctly. It says nothing about the other components of the net value of the ticket.

Wait to see if they change the schedule, before the deadline for canceling with any additional fees. If they change the schedule enough, then you can get everything back and are not charged any fee if you cancel. See this thread for details:
