The fact that it isn't automatic has to be one of the stupider, customer-unfriendly things. Basically United wastes their own and their GS/1K's time with a bunch of calls all for the "benefit" of not having to extend the plus points of GS/1Ks who don't know about the ability to extend.
Of the 130K 1K & GS customers, 5K probably extend. Imagine if the remaining 125K members saw an April 30 expiration date suddenly appear. They would call in thinking it's a mistake and completely flood Mileage Plus. UA's doing the right thing.
