This survey is open only to Starwood Preferred Guest® members 21 years or older.
Starwood Hotels & Resorts Worldwide, Inc.'s ("Starwood") use of your survey responses will be governed by Starwood's privacy policy.
Please note that the survey may be hosted by a third party vendor retained by Starwood, provided, that in such event, the third party vendor will have no right to use any data supplied by you except as necessary to fulfill its services to Starwood.
All survey responses become the sole property of Starwood.
By responding to the survey, you hereby: (a) authorize Starwood and each of its affiliated and subsidiary companies, and their respective agents, employees and others working on their behalf (collectively, the "Starwood Group") to collect, process and use all the data provided by you herein (including personally identifiable information) for any purpose that the Starwood Group deems appropriate, and specifically to store this data at such location(s) and with such party(ies) as the Starwood Group deems appropriate, whether within the United States, the European Union or otherwise, and for the Starwood Group directly, or through its third party vendor(s), to transmit whether by mail, facsimile or electronically, or by other reliable means, all or part of such data to any location that the Starwood Group deems necessary, whether within the United States, the European Union or otherwise, (b) consent to the Starwood Group's use of the data provided by you for all legal purposes, and (c) release and hold Starwood and its affiliates, officers, directors and employees harmless from any loss or damage arising in connection with participation in the survey.
The survey and Starpoints(R) bonus may be canceled, modified or suspended at any time.
There is a limit of one Starpoint bonus per person. Starpoints will be awarded within 4-6 weeks of completing the survey.
Starwood is not responsible for errors, omissions or technical problems in the survey or any related web site.
Starwood may disqualify anyone who tampers with the survey.

As I mentioned in my earlier post, I don't see any buttons to accept anything. I've tried refreshing the page and have the same internet settings as I always do (medium security). Is anybody else having this problem?
