In case anyone is interested, it looks as if I did ultimately did win this one.

AM ignored my two letters to them. They responded to my DOT complaint; they apologized for my inconvenience, but they refused to issue a refund. I submitted a dispute to American Express. American Express ruled against me. I appealed (redisputed) with American Express. AM apparently failed to respond to American Express the second time, and I won by default.
