I routinely put the same numbers in for biz card apps. Earlier this year Chase called me with questions about one. The agent wanted to know:
  • What is this business?
  • What experience do I have in this line of business?
  • What level of credit charges do I anticipate making?
  • Will my pursuit of this new business affect my current sources of income?

...All very fair question when you think about it. Especially the last one, as it's totally plausible that a person starting a new business may quit a job or cease doing some other activity that provides at least part of their stated income.
But do you also put your name as the business name, etc, or do you give an actual business name? That matters as much as the numbers.

And did they call you during the application process, or after you were approved? You also didn't explain that.

General recommendations in the Chase forum Applying thread is to not call Chase when they ask you to call about a Chase business app, and to try not to take a call from Chase during the time a business card application is in process, since they're more likely to approve you in the end if they don't talk to you while somewhat more likely to deny you if they do talk to you.
