Just to mention that in case you have a EuroBonus account and you were logged in to that account when purchasing the ticket, viewing your reservation, changing seats, during OLCI, etc... your miles may not be credited to Miles&Smiles as your EuroBonus account number would be in the reservation ( even if you typed the Miles&Smiles number during booking and even if you haven't entered your EuroBonus number at any time ).
By the way, "O" fare class refers to zero so what is your original booking class stated on the ticket?
