I am with Ivoryoutlaw (the OP, and welcome to FT to him) in this.
These days, vegetarian is hardly 'special' anymore. It was probably different 5 or 10 years ago but these days vegetarian is certainly a 'main stream' option. And in the premium cabins, it would be reasonable to offer it as a main menu choice. That of course does not necessarily mean that it will always be to the individual's taste but that goes for any meal. Of course there are several vegetarian 'streams/ types' (Asian vegetarian etc), so that does complicate things a bit so in my view 'mainstream' vegetarian (not vegan) on the menu plus other 'types' as SPML's. In my experience there often is a vegetarian option in J (or F, which is typically the J menu plus one or two additional options), but it is not consistent.
In the economy cabin, where choice is more limited to start with, it might be more difficult to achieve this, so to limit vegetarian to SPML maybe the only realistic option. (of course this may limit op-up potential but that is a story for another day).
