There really isn't a high ratio because LEOs are called in for the heavier threats. The system is designed to search for even components of a threat (IED components, weapon parts) so a screener may call a bag search and an item is cleared. These searches do not warrant an incident report.
Of weapons found, what percentage are really honest mistakes? (Those people should be caught and sanctioned, to some degree).

The average person found to have certain prohibited items will only receive a warning letter (it's procedure that certain items need to have an inspector review the report). For persons who are found to have more serious items the sanction goes from civil penalty to arrest.
For the incident described in this thread the matter would be marked as no action and the report filed.
