Formal dining etiquette is slowly dying. Not a horrible thing, but somewhat unfortunate.
I was taught these table manners, so whenever I see incorrect knife direction, or (worse) a knife on the left, I cringe a little inside haha
Setting up is one thing. Using is another. Always interesting to watch people. In different countries, different tribes. But I'll refrain from further comments.
