Use your social security number, home address and YOUR LEGAL NAME as the legal business name on the application. DO NOT make up a business name and DO NOT append anything like "consulting" to the end of your name. Sorry for the CAPS, but this is the most common area where applicants go off the rails, so I can't emphasize enough.
