I don't understand how you can think that. You can need to go to the toilet, you don't need to go to Costco. If all the disabled car spaces are used at Costco, you have many options, none of which involve wetting yourself:
Wait.
Use an ordinary car space.
Go shop somewhere else.
Come back later when it's less busy.

If the companion care toilet is being used at an airport, you have much fewer options. Waiting may mean missing a flight. Aircraft have toilets which are too small, so waiting until the flight is out. About the only option is to use the Men's or Women's and hope there is a disabled stall available inside and that nobody objects to having someone of the opposite sex in there.
