Random means random. Both as to selection and as to what the secondary inspection consists of.

If you have specific medical restrictions, you should have a letter on your doc's stationery explaning exactly what those are, e.g., "do not remove shoes" Contacting TSA ahead won't make any difference, but the letter will.

TSA can provide a chair and whatever else might be necessary.
