I thought it might help to post a link from here to a post of mine on the ITA thread which contains what I'm 95% sure is the reason for what
Collie206 sees. This problem is nothing to do with adult fares and child fares, but is because there is only one K class seat on the outbound. ITA is pricing one person using a K/K combination and the second person using an L/K combination. But most airlines and booking engines do not allow you to get this into one single booking because every passenger in the PNR must be fared using the same fare on each sector. See
