I love it when I report a personal experience, and an "expert" tells me it didn't happen. Well, here are some possibilities that may explain the differences, although they may not necessarily be a factor in my last experience.
1. The middle point of your travel has high departure tax (e.g. CDG, LHR, etc). It would only become applicable, when it's the starting point of your PNR.
2. The starting point of your PNR does not allow YQ for the whole itinerary. If you split such PNR, the 2nd half gets hosed.
