Stuff has fallen off of aircraft on occasion since aviation began. It happens less now, but it's certainly not the first time that stuff has fallen off an aircraft over a populated area.

The alternative is to have airports located in remote areas. The problem is that soon after airports are built in remote areas, developers come along and build houses around the airports. And then when stuff falls off an aircraft, it lands in populated areas rather than a cornfield.
