Take a breath. GE is not interested in outsourcing details, rather that info you provide matches info they already have - which is an indication that you are honest. I'd use the company name and address from the W-2 and then answer questions - if any - during your interview. I'd bet they won't even ask.
Thank you, this was good advice (also on the breathing!), so I filled out the online forms with company information from 1099s and W-2 tax forms from the last 5 years (which was the outsourcing company information) - basically anything that would have been in my taxes (I included both full-time and part-time 1099 jobs that I had alongside my full-time job - the times overlapped, but this was OK), and I used the phone numbers for companies that were on my W-2 or 1099 tax forms.
Got conditional approval yesterday and did my interview today already and got my traveler number after the interview. Since I am currently unemployed and listed as working part-time I did get asked how I was funding my next trip; I just told the truth that I was looking for a job and living on savings. They also asked if I had ever broken any customs laws or been arrested (answer was no). Whole process was straightforward and an overall positive experience.
