I can tell you my personal experience in Italy. Family members were traveling with us. I booked two rooms for which I paid. In each case, I got the points for both rooms but only the nights for one room. My titanium benefits only applied to the room that I occupied. Breakfast was included in the room rates for the hotels where I paid. At the hotel where I used points, I chose breakfast as my titanium amenity. The person who checked me in got permission to extend breakfast to the second room. It's possible that they felt kindly since the hotel was booked completely full, I was a returning guest, and no upgrade was possible.
