In my experience, United Club domestically is by far the most lenient when it comes to gold plus wife plus a kid.

LH I’ve seen (mostly in Frankfurt) strict enforcement (I’ve guested a kid in before).

Outstation that use contract lounges, the checkin staff will probably just write you a lounge pass for 3.
