Only MQM's count towards MM status. The miles you earn monthly from everyday spend on the DL Amex cards only earn redeemable miles, not MQM's (and thus no lifetime miles). Yes, if you spend $40K annually on the DL Reserve, you will only qualify for the first 15K MQM boost and need to spend a full $60K annually to get the second one. DL also provides a redeemable mileage boost of 15K miles when you hit the MQM boosts.
