Birthdates for lap children on already ticketed PNRs can be changed by those airlines. If there is a US-serving airline that can’t do it, it would be news to me and strike me as more likely a case of agent’s unwillingness rather than agent having no way to get it changed. Many an airline rep won’t even trust that the entered birthdate for lap children is correct unless and until shown proof of birthdate for lap-children, ticketed or otherwise, on date of travel.

The cost of traveling on an international itinerary (serving the US) with a lap-child should be higher than the cost of traveling without the lap-child, so I wouldn’t necessarily trust the vacation package pricing as being inclusive of lap-child ticket cost; and I would definitely be on the hunt for the lap child ticket number if going down that travel package route.
