For a while you could stop by a Bank One branch, report the M+ debit card lost, and re-receive the 2,500 mile bonus, 1,000 mile upgrade and $25 off coupon once you made a credit purchase using your reissued M+ debit card. It worked for me 11 times before they figured out what was happening! Its always funny to see the Bankers face when they've seen how many closed debit cards I've lost!
