Using the reporting online option, I'm stymied at the question that asks:

"The date your most recent passport book was issued"

I don't know. Is that the same date as the passport card was issued?

It also wants to know the passport book number. I don't know that.

And yes, I should have made a copy.

ETA: OK, I managed to work through the online reporting and now have the application for a replacement passport printed. (Wish they could just use the photo in the old passport)

Now to find a place to get passport photos taken.
