Fares don't really get raised to include it. In most markets fares are priced competitively, not based on costs, because that's what will get them the most revenue.

In non competitive routes they are also priced to maximize revenue, which is basically the max gouging that the customer is willing to endure.

Costs don't really play that much of a factor in either decision. This is why unbundling is so successful.
