"Pilot error" is a lazy conclusion, and never helpful.

Why was an incompetent pilot in the aircraft? Why was the aircraft unable to be flown by a competent pilot?

Who passed buddy in in the check ride? Who designed an aircraft that only immortals can control?

Who signed off on the check pilot? Who signed off on the checklist? Who signed off on the exam?

And on and on and on.
