To begin with, CDW (whether from travel insurance, Amex, or the rental car company) only covers damage to your vehicle (i.e. #1 above). It does NOT cover #2 . So, if you get the Platinum Amex, Amex will definitely NOT "step in and deal directly with [the injured party's] lawyers" in the event you're sued. To get liability coverage for #2 , you'll typically need to buy it from the rental car company, unless it's already included in the rate, which it typically will be if you book a US rental via a company's UK portal.
