You don't mention what type of tickets the relatives are on; if they happen to be on a 2-4-1, and you make a new booking for the wrong person, that won't end well.
Otherwise, a nosho with no downline space to be cancelled won't affect anything, except that the 4th person may not be able to check-in online due to someone (themself) with the same/similar name in another PNR.
