Nothing will happen. The person you'll buy an Avios ticket for will fly on that Avios ticket, while the other 3 persons in the party will fly on the original ticket. Just make sure the other 3 do not check the 4th person in using the original ticket. I also always advise to call in and advise the airline that the person won't fly.
