Of course they don't mean insulin or alcohol - they specify "illegal" drugs. What they care about is liability, period.
It wasn't an issue on our cruise; we were never searched embarking in Alaska, no matter what might have been in our coats or bags going through the screener. I simply took the photo because my cruise occurred soon after this thread started so it was in my mind, about how cruiselines might or might not be responding to the issue.
