The only time I had one of these, I was definitely told that if it didn't clear first time, it would remain available for use on a future flight. I can't say if that was correct though, as it did clear on my first attempt to use it.
Same for me. I was told to call Customer Relations a few days before the flight (which I did) and was told that I could use it again if it failed to clear. In the end, I was upgraded whilst in the lounge at T5 about 1.5 hours before departure (by Special Services, according to my boarding pass).
