I've looped HHonors support into the chain, but their email response simply supported the fact my reservation was for 3 adults, without addressing the issue of fitting 3 adults on two twin beds.

Any suggestion would be appreciated. The reservation is in November, so I have plenty of time. Thanks!

We do apologize for the confusion however this is stated that a rollway bed cost is not covered by points, please refer to the below.
So I ended up not staying at Hilton, instead opting for Conrad once again. Conrad's twin bed is 140cm each, so each bed can be shared fairly comfortably by an adult and a child aged 7-10yo.

It's getting longer and longer since the last time we stayed at Hilton in Shinjuku. But I would like to point some things out...

1). It seems like Conrad and Hilton Tokyo (Shinjuku) are pretty similar in their enforcement of rollaway fees. Award booking for 3 adults costs the same as for 2 adults, whereas there's extra cost when booking a paid stay. For the award stay, their policy is that the 3rd adult can decide to squeeze in for free into existing bedding or you have to pay the fee for the rollaway. For the paid stay, the 3rd adult makes the rate much higher but that is largely accounted for by the fact that the price of the rollaway is included. Again, this is my knowledge of Conrad's policy. So I can't say that the exact same policy holds at Hilton, but it sure seems like it.

2). Do you think the 3 of you might be able to sleep together in a 220cm wide bed? That would be about a foot wider than a US king-size bed. Honestly, it would be pushing it for 3 adults, though... unless you guys are very slim and don't move much in sleep.

3). If yes to (2), then ask Hilton if they could stick the 2 beds together. That's what would get you a 220cm wide bed. That's called Hollywood twin configuration in Jpn. I can't remember if Hilton did this for us or not. Would be nice if Conrad could do that and create a 280cm wide bed, but unfortunately they can't due to a fixed nightstand between two twin beds. Some hotels can do this, though.
