Yes, about 10 years ago. Checked in at Boston for an AA flight to St. Louis. I had pre-booked the seat, knew the gate number, and was short of time so did not bother to look at the boarding pass. Took the boarding pass from the gate agent, went through security where they "checked" the boarding pass against my drivers license as they do here for all flights. Arrived at gate and boarded the airplane, where they "checked" it again. Once I was on the plane, only then did I look at my boarding card and that was only to check my seat number. The seat number was one in first class, not what I had expected, so I looked at the boarding pass again. It was a totally different name, nothing close to mine and for a man (I am a woman). I showed the flight attendant who seemed concerned, but the gate agent could care less and just issued a new boarding pass. I did not do anything about it because I was just glad to get on the flight and go home. But it does raise major questions about so called security at our airports.
