In my experience with AA at MXP, you can arrive about 16 hours after your scheduled departure and still make the flight.

However, assuming an ontime departure I'd recommend being in the terminal no later than 90 minutes before departure. As noted above, there is a long trek to the AA check-in area, then a long trek back to security, then a long trek from the lounge to the plane.
