The night before and stay at the Sheraton MXP where you will have a short 2 minute (inside) walk to the terminal, a quick elevator ride up. Really, for flying out of MXP, this is by far the best way to go.

Given that, without baggage to check, walk out of the hotel or arrive 90 minutes beforehand would be sufficient. I wouldn't cut it closer than that because of security. If you are in J or have other means for Fast Track maybe a bit less.

ADDED: BTW - which is exactly what I am doing in early March for MXP-JFK except for 9AM flight (departure time for AA 199 moves up from current 10AM to 9AM by then)
