If you have never been there MIA is very interesting for a 24hr visit, but the airport can be a royal pain and summer in Florida is not for everybody. If you are travelling in winter then an alternative could be JFK-SJO-BOS, for both miles and an interesting destination.
Milano is great in fall/spring, problematic the rest of the year. Mileage is poor from LHR but if it is your first visit to Italy, who cares about miles?
