My sisters have been denied upgrades because they were wearing jeans. This was on Canadian though. They were on an eligible fare, certs (stickers back then I think) taken, with biz class boarding passes in hand. The gate agent taking the boarding pass at the gate denied them. I think they ended up in two crappy seats in the back.
I would have made a scene, but they were too naive I guess.
