You will have to earn 25,000 PQMs and $3,000 PQDs (unless you have a credit card waiver or international resident) by Dec 31,2018 and then you have to start all over.
You correctly understand what PQDs are -- it depends on the flights and airport involved how much the fees are. The deep discount economy flights it may only be 70% of the total fare but for me it tends to 85% or more for most of my domestic flights.
For an all UA domestic trip, the first item in the pricing calulation on the emailed receipt, "Airfare" is the PQD.
