Flying from KOA to Europe I usually go east through the US, booking on United.com, Delta,com or AA.com. They never give me options for going the other direction, even though it's equal distance. What programs might I check to find flights going the other way, connecting through Asia or the Middle East? Since there aren't that many airlines that directly service HI from the west, I'd look up which ones do so (the wikipedia entry for an airport is a good place to start). Once you know the potential routes, correlate those to which mileage programs you have available. Of course factoring in the ultimate destination in Europe, too.
Are you looking to have a stopover in Asia/ME? Or just route that way without a stopover?
