Also, sometimes individual flights are a problem. I encountered this at CO/UA, too. There will be a problem with a single passenger on the list (like a broken segment or a botched itinerary, for example), and that problem will cause the automatic upgrade computer to not run the entire flight at all. In my experience, these flights are handled manually, with lots of what we UA passengers used to call "Battlefield Upgrades" being done. Which means at the gate or even after boarding is complete.
