I have also lived by the notion that a twisted belt is not as safe as a flat one.

If I notice (and there's time before takeoff) I undo the belt from the buckle and lace it correctly.

I put it down to ignorance and lack of education of those responsible for such things.

Kids riding bikes against traffic is another example of ignorance and not being educated on the proper way to do something.
