It's one single 'fare' (ticket). Part of the Transport for London (TfL) system.
You don't go outside between Monument and Bank, it is a (rather long) walk through the maze of tunnels connecting them. I can't remember off the top of my head if / how many sets of stairs there are on that transfer.
I would support the LGW option. Easy taxi / Uber ride to Victoria and then Gatwick Express.
Very much last choice would be LHR.
