Did the mileage ticket charge look like a ticket with an itinerary (AAA-BBB) on it? If not, I'm guessing that Amex might simply consider anything that doesn't have ticket-type AAA-BBB information on it as an "incidental" charge perhaps?
However, whether it appears that way or not may depend on how it's charged. Ie, it
might appear one way when you're booking an award online, a different way when you're booking an award over the phone, a different way when you're modifying an award (to one which has higher taxes/fees), etc. So just because it worked in one case does not necessarily prove that it works in all cases.
